The company brings a highly clinical and data-driven approach to MSK care, with a solution focused on 3 main pillars: care planning, care delivery, and care navigation. The system is designed to predict and qualify clinical risk factors to highlight where the biggest impact is going to be. From there, Livara is able to take the continuous flow of data from the patient to the clinician and quickly adjust any treatment course based on these results. 

Livara is pioneering value-based contracts in the MSK space, headed by CEO Rob Cohen and Co-Founder and Chief Medical Officer (CMO) Kamshad Raiszadeh. Unlike most competitors, the company takes on risk and focuses on Medicare and fully insured populations, with key partners that include Elevance, Providence, and Scripps.

Job Summary: 

The Physician Assistant/Nurse Practitioner position supports Livara Health Medical Group by serving as a treater as well as a liaison between the physician(s), the in-clinic Physical Therapy Team and the virtual Exercise/Wellness Team. In this role, the Nurse Practitioner will participate in Therapy Rounds as well as Medical Rounds and will take a proactive role in preparing patients for one-on-one physician consultations and following up with recommendations.

Supervisory Responsibility:

There is no supervisory responsibility for this position at this time.

Essential Job Functions:

  • Ensuring all required information is available and summarized for the physician in advance of one-on-one patient calls/telehealth visits
  • Using dashboards to evaluate patient progress through online and in-clinic services
  • Assess high risk patients and one’s not progressing them to guide their care
  • Attending physician/patient calls (on an as needed basis and time permitting)
  • Pre-rounding for clinics including reviewing patients submitted for rounds
  • Attending both clinic-based and virtual client-based rounds
  • Working in coordination with the Care Navigator to ensure appropriate radiographic studies / reports and other pertinent medical records are obtained
  • Ability to access and measure radiographic studies, if necessary
  • Coordinating asynchronous Therapy Rounds with PT Leadership and asynchronous Medical Rounds with Physician Leadership including review of cases and communication of plan of care back to coach, primary therapist and/or Care Navigator
  • Demonstrates clinical knowledge and skill in assigned area and within scope of practice
  • Monitors patient flow, physician schedules, and completes daily tasks to ensure quality and meet service standards
  • Able to respond to changing circumstances and prioritize clinician and patient needs
  • Documents patient care events in a thorough and accurate manner
  • Assists research team with data mining and chart review
 

Required Experience:

  • 2-4 years of experience required in clinical setting (preferably in MSK and Spine)

Required Education:

  • Licensure requirements of the state of jurisdiction - AZ (required), CO, WA, TX, MA, GA, NC (preferred)
  • Graduation from an accredited Nurse Practitioner or Physician Assistant program
  • Master's Degree in Nursing or related Health Care degree or equivalent experience
  • California Registered Nurse (RN) - CA Board of Registered Nursing -REQUIRED
  • California Nurse Practitioner (NP) Certificate - CA Board of Registered Nursing -REQUIRED

  • Demonstrate typing skills proficiency by: Using a keyboard, required to type proficiently and accurately; Have the ability to type a minimum of 60 words per minute with 0-2 errors; Have the ability to proofread work

  • Remote work environment 
  • Prolonged periods of sitting or standing at a desk and working on a computer
 

Travel:

This role will be both remote and at company locations as needed, with some travel occasionally.

 

Compensation Range, DOE (Salary, Exempt):

$124,000 - $133,000
